Who is responsible?
The Editors' Committee make the Code Rules.  The Press Complaints Commission enforces it.

When is it appropriate?
For most print media.

"It is the responsibility of editors and publishers to implement the Code and they should take care to ensure it is observed rigorously by all editorial staff and external contributors, including non journalists...” PCC Code.

You can use the PCC Code to get an apology and correction.

How can I use it?
Quote the following:

1.Accuracy
◦"The press must take care not to publish inaccurate, misleading or distorted information, including pictures.
◦A significant inaccuracy, misleading statement or distortion once recognised must be corrected, promptly and with due prominence, and - where appropriate - an apology published." PCC Code
